Long Term Conditions
Sandgate Road Surgery employs a team of highly qualified and experienced Health Care Assistants, Nurses and Specialist Nurses who work closely with our GP’s to meet your health needs.
Our GP’s and nurses are committed to playing a key role in managing your long-term conditions. We believe in a collaborative approach, working closely with you and community specialists to deliver comprehensive, personalized, and high-quality care.
Long-term conditions, also known as chronic diseases, are those that cannot be cured but can be managed with medications and other treatments.
All patients with a long-term condition should have an annual review and the practice proactively contacts patients to ensure they are seen by the appropriate Clinician as required. A list of the Long Term Conditions we treat are:

If you are concerned that you have not been called for your annual review please speak to our receptionists who should be able to advise you when your next review is due.
Family Planning and Contraceptive Care
We offer appointments for a range of appointments including comprehensive care and education regarding most types of contraception and sexual health. As well as regular checks for patients on oral or injectable contraception.
Learning Disability Health Checks
We offer learning disabilities health checks to help individuals reduce their risk of heart disease, stroke, diabetes and kidney disease.
Travel Vaccinations
As well as offering travel vaccinations available via the NHS our nurses can advise on other travel vaccinations you may need to obtain from private travel clinics.
Baby and Child Immunisation Service
Our nursing team provide a full range of baby and child immunisations.
Other Services
Some of our nurses and doctors have special interests and have obtained the relevant knowledge, skills and experience to allow them to provide certain services that are not available at every GP Practice.
