All home visit requests are triaged by a Clinician by telephone in the first instance and a home visit is only arranged if assessed to be appropriate at this time. Alternate advice or treatment may be offered.
Home visiting is undertaken by either an advanced clinical practitioner based at the surgery or an advanced practitioner or paramedic based with the primary care network team. Capacity is limited with both services, and once capacity has been reached further requests will be directed to 111 services.
